What is the Heart Health Program?

The Heart Health Program is a FREE program which aims to help you increase your physical health and wellbeing through practical exercise, nutrition and lifestyle management support. The program is delivered by an experienced Exercise Physiologist at Sydney West Sports Medicine, in groups of approximately 10-15 participants.

The program runs for 52 weeks and includes regular physical activity sessions (twice/week) tailored to your needs and fitness level and 12 health education modules. The program covers a range of topics including setting healthy goals, nutrition and healthy eating, physical activity, chronic conditions, quitting smoking, responsible alcohol consumption, back care, stress management and maintaining a healthy heart.

The program is offered in two formats – the Heart Health Group Program and the Individual Heart Health Program.

Why should I register for the Heart Health Program?

Physical health is vital to your mental health and your overall quality of life. There are a number of benefits to a daily routine of exercise and balanced nutrition.

Most people want to improve their health but find it hard to get started. Registering for Heart Health gets you started and keeps you going on the path to new lifelong health habits. Changing our routines to improve our health can be challenging. The Heart Health Program gives you the skills to improve and sustain your health and wellbeing by giving you practical information and access to specialist advice and training.

Eligibility

The Heart Health Program is FREE and open to all veterans and peacekeepers who have not previously participated in a Heart Health Program. All participants require medical clearance from their GP before beginning.

DVA Entitled Rehabilitation Services

The Department of Veterans’ Affairs (DVA) provides a wide range of health services for veterans, war widows/widowers, and dependents entitled to clinically required treatment for their accepted health conditions under the Repatriation Medical Benefits Schedule.

What type of health services are funded by DVA?

DVA funds a range of health services that are clinically required to treat your particular health conditions. General medical services are usually delivered via your General Practitioner (GP), however your GP, treating hospital doctor, or hospital discharge planner may also refer you to a qualified specialist practitioner for specialist medical treatment of your accepted conditions.

What is the aim of the DVA funded program?

The aim of the DVA funded health services is to assess the entitled person and design an individualised treatment/rehabilitation program specific to your referred clinical condition, and work towards providing the entitled person with the skills to manage their treatment independently on a long-term basis.

Who can provide Treatment?

Treatment services must be provided by an Allied Health Practitioner who is board certified and registered with the Department of Human Services. The Practitioners at Sydney West Sports Medicine are all appropriately accredited and trained to provide expert advice and optimal care of eligible DVA patients. We have unlimited access to start of the art facilities to assist in your rehabilitation and recovery including a hydrotherapy pool, rehabilitation gym, and Sydney Gymnastics and Aquatic Center (SGAC) which enable our practitioners to provide a multi-faceted approach to patient treatment and rehabilitation.

How can I receive treatment?

You will need to have:

  • A referral from a Medical Practitioner, Medical Specialist or Hospital Discharge Planner, which includes a diagnosis informing the Practitioner of the Patients Condition requiring clinically necessary treatment

  • A Gold Card for all conditions or a White Card where the referral for treatment is for a war caused or service related condition accepted by DVA.
